Policy on Multicenter Retrospective Data Sharing

If you wish to conduct multicenter research studies that involve sharing of de-identified retrospective data, please follow these instructions:

• Each site must complete the Deidentified Data Submission for Multicenter Research Form.
• The principal investigator at SLUHN is responsible for sending this form to all participating centers and must maintain a record of the emails sent for documentation purposes.
• The principal investigator must retain the Deidentified Data Submission for Multicenter Research forms as part of his/her study records.

Note: This form only applies to de-identified retrospective data. All other types of data sharing involving sites outside of SLUHN must have a Data Use Agreement (contact Jane Markson, Esquire, for more information—Jane.Markson@sluhn.org).
