Home Health

St. Luke’s Home Health professionals provide care and education to you and your family members as you recover from surgery or manage an illness. Our care is medically directed and delivered to you in your home. Services are provided under a plan of treatment established and approved by your physician. Services generally include the skills of a registered nurse.

Hospice

Most hospice care is given at home. Facing a life-limiting illness is difficult and has many challenges for both the individual and for the family. St. Luke’s Hospice brings a team of experts to help you. The team is comprised of physicians, registered nurses, social workers, chaplains, hospice aides and volunteers to assist you and support your family through your journey.

VNA of St. Luke’s Nurse-Family Partnership

The Nurse-Family Partnership (NFP) is a voluntary home visitation program where specially trained Registered Nurses visit low-income, at-risk mothers. These visits start early in pregnancy and continue through the child's second birthday.

Events

St. Luke’s VNA holds a variety of events throughout the year to educate the community about the services we provide, lend support to our patients and their families and raise money to ensure we are able to continue providing compassionate, end-of- life services to all patients, regardless of their ability to pay.
