Visiting Nurse Association
Quality care focused on you.
For more than 100 years, the VNA is here for you.
The Visiting Nurse Association (VNA) is a dedicated healthcare organization that provides comprehensive home health and hospice care to patients within the community. Our experienced healthcare professionals deliver compassionate and skilled care to individuals of all ages, allowing them to heal and thrive in the comfort of their own homes. Whether you require short-term rehabilitation, ongoing chronic care management, or end-of-life support, our VNA team is committed to providing personalized care tailored to your specific needs.
Home Health
St. Luke’s Home Health professionals provide care and education to you and your family members as you recover from surgery or manage an illness. Our care is medically directed and delivered to you in your home. Services are provided under a plan of treatment established and approved by your physician. Services generally include the skills of a registered nurse.
Hospice
Most hospice care is given at home. Facing a life-limiting illness is difficult and has many challenges for both the individual and for the family. St. Luke’s Hospice brings a team of experts to help you. The team is comprised of physicians, registered nurses, social workers, chaplains, hospice aides and volunteers to assist you and support your family through your journey.
Maternal Child Health Programs
Empowering families from pregnancy through childhood, the VNA of St. Luke’s Maternal Child Health Programs provide personalized support, education, and resources to build healthy and thriving families. Explore our programs including Nurse-Family Partnership, Moving Beyond Depression, and the Visiting Nurse Advocate for the County to see how we make a positive difference in the lives of parents and children.
Events
St. Luke’s VNA holds a variety of events throughout the year to educate the community about the services we provide, lend support to our patients and their families and raise money to ensure we are able to continue providing compassionate, end-of- life services to all patients, regardless of their ability to pay.
