The price listed on this site INCLUDES the following:

For Procedures:

• The costs associated with the use of the operating room/procedure room
• The cost for the physician to perform the procedure
• The cost of any image/X-ray performed during the procedure along with cost to have the radiologist read the study and provide a report.
• The cost of anesthesia services
• The cost of the post-op visit at the physician office

For Imaging or Testing:

• The cost of the image/x-ray/testing at the hospital or testing center
• The cost of the physician’s interpretation of the image/x-ray/testing and providing a report on the study when applicable.

For Lab:

• The cost of the lab test at the testing location
• For Physical Therapy and Occupational Therapy Evaluation:
  • The cost of a Physical Therapy or Occupational Therapy Evaluation.

For Rehabilitation:

• The cost of the physical or occupational therapy at the therapy site

Because the human body is different for everyone, additional procedures, imaging studies and/or testing may be needed to meet your specific care needs. This pricing DOES NOT INCLUDE any additional procedures, imaging studies and/or testing beyond what was originally ordered and paid for. Costs of care resulting from special circumstances during or following the completion of the procedures, imaging studies and/or testing are not included.

Also not included are visits with your physician prior to the surgery, lab specimen collection and analysis, any unusual implants/hardware or special devices, special pre-procedure services such as lab tests, physical therapy, EKGs.

The prices listed are not negotiable and are available only to those who pay the entire amount in advance.

This program does not apply to patients who have governmental insurance (Medicare, Medicaid, and managed Medicare/Medicaid).
